One Leisure Event
Personalised Care Team supports carers and people with learning disabilities at One Leisure event
Carers and families supporting people with learning disabilities were welcomed to a community event at One Leisure on Friday, 21 November 2025. The event was supported by the North Integrated Neighbourhood Team for St Neots, where our Personalised Care Team joined other local partners to showcase the support available for residents across St Neots.
Throughout the day, the team including Social Prescribers and Care Coordinators met with attendees to talk about the personalised care support offered locally. This included guidance on getting involved in community-based activities, accessing services that support social and emotional wellbeing, and understanding care options to ensure people receive the right help at the right time.
The team also outlined the support available for residents living with long-term health conditions or frailty, as well as anyone who may need extra help to stay independent. They spoke about Carer Conversations, which provide personalised advice, practical information and tailored guidance to help carers find the support that works best for them.
Lucinda Best, Care Coordinator at St Neots Primary Care Network, said: “Carers do an incredible job, often while balancing their own pressures and responsibilities. Events like this give families a chance to talk openly, ask questions and find local support they may not have known existed. Even a short conversation can help someone feel more confident and less alone.”
The event forms part of wider work taking place in St Neots to make local services easier to access for carers and people with learning disabilities, and to ensure local community groups continue to play a vital role in helping residents stay connected and supported.
If you are a patient and feel you would benefit from personalised care support, please speak to us to request a referral. This is a non-urgent service. Referrals are reviewed within seven days, and a member of the Personalised Care Team will contact you to discuss your needs and next steps.
