This service is for patients who would like more choice and control over their health and care.
Our team
Personalised care starts with a conversation about what matters most to you. It helps you plan care and support around your life, goals and priorities.
As your local Personalised Care Team, we work alongside your GPs, nurses, and other healthcare professionals to provide joined-up support for your health and wellbeing.
Our team includes:
- Social Prescribing Link Workers
- Care Coordinators
We’ll help you access local services, practical advice, and community support to help you stay well and independent.
What can we help with?
Social Prescribers
We support adults (aged 18 and over) who may need help with:
- Bereavement or loss
- Caring responsibilities
- Day-to-day support
- Employment or financial worries
- Food or housing problems
- Loneliness or isolation
- Living with long-term conditions
- Low to moderate mental health issues
- Substance misuse
- Transport
- Abuse or domestic violence
We can link you to local community groups, charities, and services that can help you feel more supported and connected.
Care Coordinators
We support people who are living with long-term conditions, are frail, or need extra help to stay independent. We can help with:
- Managing a long-term condition
- Mobility or frailty issues
- Caring responsibilities
- Accessing care and support services
- Information about retirement or care homes
- Referrals to Adult Social Care or physiotherapy services
- Dementia support – including advice, practical help, and links to specialist services
- Helping you stay well at home and avoid unnecessary hospital visits
Support for Carers
If you care for someone who couldn’t manage without your help, you are a carer and support is available for you too.
A carer’s assessment gives you the chance to talk about your caring role and how it affects your life. It helps identify what support you can receive.
We offer Carers Conversations to help you get the advice and support that works best for you.
How to access our services and support
If you would like support from our team, please speak to your GP practice to request a referral.
We provide a non-urgent service. Referrals are reviewed within seven days, and a member of our team will contact you to discuss your needs and next steps.
